Job Description
ChristianaCare HomeHealth is looking for experienced Home Care Licensed Practical Nurses (LPNs) to join our supportive and innovative team. As a Home Care LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and collaborate in developing discharge plans.
Responsibilities
- Provide skilled nursing care to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
