Job Description
We are looking for experienced Home Care LPNs to join our ChristianaCare HomeHealth team. As a Home Care LPN, you will be responsible for providing skilled nursing care to geriatric patients in the community, implementing care plans, and participating in discharge planning. This is a Full Time, Part Time, and Per Diem position with varying work hours, offering competitive pay starting at $22.00 per hour.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High school diploma or GED required
- Current DE LPN license or compact state license
