Job Description
We are seeking dedicated Home Care LPNs to join our team at ChristianaCare HomeHealth. As a licensed practical nurse, you will provide skilled nursing care and treatment to geriatric patients in the community. You will play a crucial role in implementing care plans and ensuring patient well-being. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
