Job Description
We are looking for experienced Home Care LPNs to join our ChristianaCare HomeHealth team. As a member of our dedicated staff, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, develop patient-centered care plans, and contribute to discharge planning. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
