Job Description
We are looking for an experienced Home Care LPN to join our ChristianaCare HomeHealth team. In this role, you will provide skilled nursing care and treatment to geriatric patients in the community while supporting the development of patient-centered care plans. Responsibilities
- Deliver skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
