Job Description
ChristianaCare HomeHealth is seeking experienced Home Care LPNs to join our team. Our organization is well-prepared for Oasis-E, with a state-of-the-art Electronic Health Record system and comprehensive OASIS-E training. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented, patient-centered, and consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
