Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in coordination with available resources.
- Contribute to discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
