Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare HomeHealth. As a member of our organization, you will have access to top-notch electronic health records and comprehensive training in OASIS-E. Our supportive leadership team is dedicated to helping you succeed in your Home Health career.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Contribute to discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
