Job Description
ChristianaCare HomeHealth is seeking compassionate and experienced Licensed Practical Nurses (LPNs) to join our dynamic team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning.
Responsibilities
- Give skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- Must have a High School Diploma or GED.
- Current DE LPN license or compact state license.
