Job Description
We are looking for passionate and experienced Licensed Practical Nurses (LPNs) to join our dynamic team at ChristianaCare HomeHealth. Our organization is dedicated to providing exceptional care to our patients, and as an LPN, you will play a crucial role in delivering skilled nursing care and treatment to geriatric patients in the community.
Duties and Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
