ChristianaCare Home Health is seeking Full Time, Part Time, and Per Diem LPNs to support New Castle County, DE. Duties and Responsibilities:
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented, patient-centered, and resource-consistent.
- Participate in developing discharge plans for patients.
Requirements:
- HSD or GED required.
- Must possess a current DE LPN license or compact state license.
