Job Description
ChristianaCare HomeHealth is looking for dedicated LPNs to join our team and provide top-quality care to our patients in New Castle County, DE. As an LPN in our Home Care program, you will deliver skilled nursing care and treatment to geriatric patients, work with care plans, and contribute to discharge planning.
Responsibilities
- Provide skilled nursing care to geriatric patients in the community.
- Implement patient-centered care plans within available resources.
- Participate in developing discharge plans.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
