Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join ChristianaCare's HomeHealth team. As an LPN in our organization, you will provide skilled nursing care to geriatric patients in the community, collaborate on care plans, and contribute to patient discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
