Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our dedicated team at ChristianaCare HomeHealth. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning. This position offers flexibility with full-time, part-time, and per diem roles available.
Duties and Responsibilities
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented, patient-centered, and consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
Job Types

Compensation
Pay: $22.00 - $28.00 per hour
Expected hours: 20 – 40 per week
