Job Description
We are seeking enthusiastic and skilled Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN, you will have the opportunity to provide compassionate care to our geriatric patients in the community. Our Electronic Health Record system is state-of-the-art, and we offer comprehensive OASIS-E training to support your professional development.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in collaboration with the healthcare team.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
