Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. This is a rewarding opportunity to provide skilled nursing care to geriatric patients in the community, implementing goal-oriented care plans and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
