Job Description
ChristianaCare HomeHealth is seeking dedicated Licensed Practical Nurses (LPNs) to join our team and provide exceptional care to patients in the community. As an LPN in our Home Health division, you will have the opportunity to work full-time, part-time, and per diem shifts in New Castle County, DE. Our LPNs play a crucial role in delivering skilled nursing care, implementing care plans, and participating in discharge planning for patients.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license, or compact state license, is mandatory.
