Job Description
We are looking for a dedicated Licensed Practical Nurse (LPN) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to patient discharge planning.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
