Job Description
We are seeking skilled Licensed Practical Nurses (LPNs) to join our dynamic HomeHealth team at ChristianaCare. As an LPN, you will provide expert care to geriatric patients in the community, implementing patient-centered care plans and contributing to discharge planning.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
